We would like to personally thank you for your loyalty to Sensible Choice products and your participation in the Consumer Rewards Program. We regret to inform you that we are discontinuing the Sensible Choice Consumer Rewards Program as of December 31, 2008. But, there is still time to collect your UPC codes from Sensible Choice bags and your original store register receipts. As a reminder, here’s what you need to do to participate in the Sensible Choice Consumer Rewards program: Purchase a total of 10 Sensible Choice products no later than December 31, 2008 (as dated on your original store cash register receipts).
Submit your completed form, 10 original UPC codes from Sensible Choice bags, and up to 10 original store cash register receipts with your Sensible Choice purchases circled to:
